Acne

Acne is such a common skin condition that everybody suffers from it at least once in their life regardless of age, lifestyle or skin type. An important function of the skin’s oil glands is to help keep the skin moisturised and supple. However, acne and acne-related conditions occur when these glands overproduce excess/thickened oil which when combined with dead skin cell debris can block these ducts, making it an ideal environment for bacteria to breed. A combination of factors can cause this overproduction including improper hygiene, environment, diet and nutrition, lifestyle, stress, genetics and hormonal changes. Acne can appear on any part of the body most commonly on the face, neck, chest, back, shoulders and upper arms.

There are four different grades of acne:

Grade 1: Congestion – Blackheads and Whiteheads

Commonly seen in teenagers but can also affect adults, especially those with oily skin.

Blackheads, or open comedones, occur when excess oil combines with dry skin and debris to block open pores on the surface of the skin. As a result of exposure to the air, the surface of the pore oxidises and turns a dark brown or black, hence the name blackhead.

Whiteheads, or closed comedones (also known as Milia), occur in a similar manner to blackheads however the difference is the clogged pore is covered by layers of skin cells. Because there is no exposure to the air, there is no oxidation and hence the clogged matter remains white.

Grade 2: Mild Acne – Blackheads and Papules with no Nodules

Papules are small, inflamed, bumpy and solid lesions usually less than 1cm in diameter. Usually pink, brown, purple or red in colour and they can be tender to touch. They do not contain puss, are usually found in clusters, and can sometimes be found alongside rashes. If scratched, they may open and become infected.

Grade 3: Moderate Acne – Blackheads, from a few to several inflamed pustules and papules, with a few to several nodules.

Pustules are usually red at the base and have a hard, inflamed or puss-filled inner region.

Nodules are large lesions, extremely painful to touch and occur deep within the skin. They are a small group of cells, and typically formed in response to an injury. If they occur close to the skin surface they are usually benign and painless, whereas if they are deep within the tissue they can be extremely painful.

Grade 4: Severe Acne – Severe inflammation with many lesions, cystic papules/pustules, and from a few to many nodules.

Cysts are usually several highly inflamed puss-filled regions that are painful and occur deep within the skin. They are especially serious as they appear as large, unsightly, and painful red bumps which if not treated correctly, can carry the highest risk of scarring. One should avoid touching them as they must be handled with extreme care.
