Acne Scarring

Acne Scarring occurs when acne has been left untreated for a prolonged period of time or has just not been treated effectively, and thus begins to affect the deeper layers of the skin. It can also result from acne that has been constantly picked at or squeezed, and with some people it can even occur from minor acne. The acne becomes infected causing inflammation, trauma and break down of connective tissue which results in scar tissue forming in the affected area. Contrary to popular belief, acne scarring can affect both teenagers and adults of any age. Such scars can be carried for life, affecting the person’s appearance and self-confidence.

Acne scarring is first classified by type, and then by grade.

Five different types of scarring:

  • Rolling: Usually rounded and indented with sloping edges. The combination of several of these scars in an area of skin gives it a rolling or wave-like appearance.
  • Boxcar: More clearly defined and steeper edges. Deeply indented appearing as round or oval pitted scars.
  • Keloid: Known as hypertrophic scars, these occur when excess scar tissue forms at the site of an injury. Appear as raised scars that extend beyond the original wound.
  • Ice-pick: Often deeply indented and appear as deep narrow pitted scars. The skin looks as if it has been pierced by an ice-pick or a sharp instrument.
  • Pigmentation: This is not a true scar and is referred to as post-inflammatory hyper-pigmentation.

Four different grades of scarring:

  • Grade 1: Mostly pigment-based. Ordinary scars from cuts to the skin normally appear white, while Grade 1 acne scars or lesions are darker or hyper-pigmented appears darker than the surrounding skin, giving the skin a visibly inconsistent skin tone.
  • Grade 2: Mild rolling scars visible at distances of less than 50 cm, and able to be covered by makeup or cosmetics.
  • Grade 3: Not easily covered by cosmetics. Visible moderate rolling scars, shallow boxcar scars and hypertrophic scars.
  • Grade 4: Cannot be covered and are highly visible. Include ice-pick, keloid and severe boxcar scars.
